Today (Monday 20 February) we launch our new Life Stage Service Frameworks, which set out how our services will support patients at each stage of their lives.
We are ambitious about what we can and will do better for our patients, our communities, and our staff and volunteers. The Frameworks will guide our work and explain how we will deliver on our objectives and achieve our vision of providing excellent care at the heart of the community.
There are three frameworks that look at key stages in people's lives:
- Starting well - Laying the foundations for a healthy life
- Living well - Ensuring that people have the opportunity to live a healthy life
- Ageing well - Supporting people to age healthily throughout their lives
The Frameworks have been created by our clinical teams using feedback received from patients through a variety of means, including the Friends and Family Test, complaints and compliments.
We're at the start of our journey but will be sharing updates and examples of the changes we're making to meet the needs of our local communities.
