Audience
The video has been produced with GPs as the key audience in mind but has been carefully designed so anyone can watch it and understand the benefits of Proactive Care.
Objectives
The video demonstrates the value of the multi-agency and multidisciplinary work of our local proactive care teams, who support local people to manage their health and care needs effectively, enabling people to stay well and live independent lives in their own homes for longer, and avoid the need to attend hospital.
The video aims to communicate the value of Proactive Care and the benefits related to better patient outcomes, better professional working relationships, to maximise GP engagement of Proactive Care across West Sussex and to support the system effectively by avoiding admissions to hospital.
Content
The video includes an interview with new Proactive Care patient Peggy Minor. She has been known to community nursing for some time, has just returned home following a stay at hospital and would benefit from an agreed Proactive Care contingency plan. The plan is agreed with Peggy and all professionals involved in her care, including her carers, so everyone knows what to do should Peggy suddenly experience a deterioration in her condition, with the aim to stabilise and keep Peggy at home.
