In June NHS High Weald Lewes Havens Clinical Commissioning Group (CCG) announced that through a competitive bid process we were successful in winning the contract to provide adult community services in the High Weald Lewes Havens area of East Sussex from 1st November.
We've been working together with the CCG to ensure safe transfer of the services and we're delighted that we're now providing the following adult community services in the area:
We provide care in peoples own homes, work closely with local GPs, and are providing community services at the three community hospitals: Crowborough War Memorial Hospital, Lewes Victoria Hospital and Uckfield Community Hospital, as well as care in the communities of Heathfield, Newhaven and Peacehaven.
Dr Sarah Perry, clinical lead for community services at the CCG said “We are confident that we will have a broad range of services designed to meet the needs of our population with our community hospitals at the heart of these services. We are looking forward to future developments and improvements in the services alongside on going engagement with patients and staff.”

A key part of our bid is the development of our local Communities of Practice (CoP) - bringing primary and community care together, working closely with local GPs. CoP are based around local GP populations and will provide community services in people's own homes, at the three community hospitals - Crowborough, Lewes Victoria and Uckfield, and at local clinics/health centres and hospices.
