We're now providing adult community services in High Weald Lewes and Havens

In June NHS High Weald Lewes Havens Clinical Commissioning Group (CCG) announced that through a competitive bid process we were successful in winning the contract to provide adult community services in the High Weald Lewes Havens area of East Sussex from 1st November.

We've been working together with the CCG to ensure safe transfer of the services and we're delighted that we're now providing the following adult community services in the area:

Services we're providing

  • District/community nursing.
  • Minor injuries units - Crowborough, Lewes and Uckfield.
  • End of life care.
  • Intermediate care beds - Crowborough, Lewes and Uckfield.
  • Specialist nursing.
  • Community diagnostics.
  • Community dietetic.
  • Community heart failure.
  • Community neurological rehabilitation.
  • Community occupational therapy.
  • Community respiratory service.
  • Continence.
  • Speech and language therapy.
  • Tissue viability service.

We provide care in peoples own homes, work closely with local GPs, and are providing community services at the three community hospitals: Crowborough War Memorial Hospital, Lewes Victoria Hospital and Uckfield Community Hospital, as well as care in the communities of Heathfield, Newhaven and Peacehaven.

Dr Sarah Perry, clinical lead for community services at the CCG said “We are confident that we will have a broad range of services designed to meet the needs of our population with our community hospitals at the heart of these services. We are looking forward to future developments and improvements in the services alongside on going engagement with patients and staff.”

Our teams have been working hard to ensure safe transfer of the services for our patients and for staff who are joining our SCT family. We’re working together with the CCG, our partners and other local providers to improve health and care outcomes for the people of High Weald Lewes Havens.”

We're looking forward to continuously improve services in the area and we've created Sussex Healthcare Alliance, working together with a number of key delivery partners who bring an enhanced range of skills to provide better health and care outcomes for local people.

We're also working very closely with other local NHS provider partners including Brighton and Sussex University Hospitals NHS Trust, Maidstone and Tunbridge Wells NHS Trust, East Sussex Healthcare NHS Trust and Sussex Partnership NHS Foundation Trust to ensure seamless pathways of care.

Communities of Practice

A key part of our bid is the development of our local Communities of Practice (CoP) - bringing primary and community care together, working closely with local GPs. CoP are based around local GP populations and will provide community services in people's own homes, at the three community hospitals - Crowborough, Lewes Victoria and Uckfield, and at local clinics/health centres and hospices.

We'll have four CoPs covering the following geographical populations:

  • Crowborough.
  • Havens - East Saltdean, Newhaven and Peacehaven.
  • Lewes.
  • Uckfield.
