We launch new ‘Diabetes Care for You’ service

Our new community diabetes service, named ‘Diabetes Care for You’, is now available to people aged 18+ who live in Brighton & Hove and the High Weald, Lewes and Havens area of East Sussex.

This new consultant-led service is available for adults living with Type 1 and Type 2 diabetes and helps people to live well by offering advice and support to manage diabetes and meet personalised goals.

Our service provides diabetes care in the community with clinics available in Brighton, Crowborough, Hove, Newhaven and Uckfield. Emotional and psychological care is available to support self-management and improve wellbeing. Specially trained Patient Care Advisors are available to help people with any queries they may have.

Richard Quirk, Medical Director at SCFT said “We’re delighted to be providing our new community diabetes services, Diabetes Care for You, in Brighton & Hove and the High Weald Lewes Havens area of East Sussex. Our teams have been working hard to ensure safe transfer of patients who previously would have been cared for at their local acute hospital. We’re working together with our commissioners, GPs and other local providers to improve health and care outcomes for people living with diabetes.”

Brighton and Hove CCG Chief Clinical Officer, Dr Christa Beesley, said: “Diabetes care has improved significantly in recent years but there is still scope to improve health outcomes for patients and reduce future prevalence of the disease. This new service model aims to concentrate care and support on the specific needs of each patient so that local people with diabetes can stay as well as possible for as long as possible.”

High Weald Lewes Havens CCG Chair, Dr Elizabeth Gill, said: “We have an ageing population in our area and diabetes is a growing concern. For the first time we are bringing all community based services for diabetes together, from prevention to treatment, with the intention of providing more integrated care and better outcomes for local people with diabetes.”

The service is provided by Sussex Community NHS Foundation Trust and is commissioned by NHS Brighton and Hove Clinical Commissioning Group (CCG) and NHS High Weald Lewes Havens CCG.
