What does this service provide?

Who we are

Our West Sussex Diabetes Community Team provides joint clinic support at GP surgeries alongside the practice nurses to adults aged 18+ living with diabetes.

What we do

When attending your GP surgery, you will be seen by the practice nurse alongside a diabetes specialist nurse, who will support the practice nurse with a plan for you once an assessment has been undertaken. You will be informed of treatment options and a decision will be made regarding the next steps.


How can I access this service?

Coastal West Sussex: Our service is available to patients living with Type 1 Diabetes and registered with a GP in the Coastal West Sussex area, except for some practices in the Lancing, Steyning, Billingshurst and Henfield areas – these practices are supported by their GP only.

Mid Sussex: Our service is available to patients living with Type 1 Diabetes and registered with a GP in Hassocks, Hurstpierpoint, Burgess Hill, Haywards Heath, and East Grinstead. There is no provision for Crawley or Horsham GPs.

GPs and practice nurses can book the joint clinic slots that they hold and then pass over a referral to the diabetes nurse in advance of your appointment.

Community nurses can refer patients directly for advice and guidance.

Our referral criteria for Type 2 patients include those who need escalation of their treatment, for example starting injectable therapy. We also provide Type 2 Diabetes Education Groups.

Opening hours

Our clinics run the same opening hours as your GP surgery. Our telephone lines are open from 9:00am to 4:00pm.

Waiting times

The practice nurse at your GP will book an appointment when they are available – our diabetes specialist nurse attends each month. Please contact your GP surgery for more information.
