What does this service provide?
Who we are
Diabetes Care For You brings together consultants, diabetes specialist nurses, dietitians, podiatrists and psychotherapists, all of whom specialise in care for adults 18+ living with diabetes. Depending on your needs, you may be seen by several of our health care professionals. We have close links with hospital-based diabetes services to ensure that if needed, you will have access to all of the services and further specialist clinicians your condition requires.
What we do
We work with people living with diabetes to explore health needs and to help understand and manage your diabetes in the best way possible. According to your need and preference we can offer 1-2-1 appointments in person, by phone or by video call. We also offer group education sessions either in person or by video. During your appointment you will be informed of the available treatment options and a decision will be made regarding the next steps.
All patients living with Type 1 diabetes should expect to have appointments with their consultant and specialist nurse throughout the year.
Patients with Type 2 diabetes will predominantly be supported by our specialist nurses and dietitians but may see a consultant if necessary.
Foot care for people living with diabetes is really important. Our podiatrists can help catch issues at an early stage and prevent future complications. You may be placed under the care of our podiatrists if your feet are assessed as moderate or high risk, or you have an active foot ulceration.
If necessary, you may also see a dietitian or be offered group education. Dieticians can help you control and improve your blood sugar levels as well as give you more confidence and knowledge to manage your diabetes.
As we know that living with diabetes can be really difficult, our psychotherapists offer time-limited mild to moderate emotional support. Some of the presenting difficulties may include adjusting to a diagnosis of diabetes, managing medication and needles, relationship to food and eating behaviours, anxiety around hypoglycaemia, diabetes distress and burn out, sexual difficulties, low mood.

Appointment types
We offer appointments in person, by video or telephone. We also offer educational groups for people living either with Type 1 Diabetes or Type 2 Diabetes. Our groups are small and friendly either in person or by video.
Who you'll see
Depending on need you'll be seen by any one or more of our team. You could be seen by a specialist nurse, a consultant, a podiatrist, a healthcare assistant, a dietician and/or a psychotherapist. You'll also be regularly in contact with our administrators either by phone or email.
How can I access this service?
Patients need to be registered with a Brighton and Hove or High Weald, Lewes and Haven GP to be eligible for our service. We need a referral from your GP (via e-referral service) or any other health care provider which can be emailed to us.
Our referral criteria:
Type 1 patients
All patients in the Brighton and Hove and High Weald, Lewes and Haven area except those who are pregnant, have had a transplant, have an Estimated Glomerular Filtration Rate (eGFR) below 15 or have HIV.
Type 2 patients
Anyone who is on the maximum dose of tolerated oral medication for their diabetes, and/or needs initiation on injectable therapy.
Podiatry patients
Patients who are at moderate risk, at high risk or have active foot problems. Patients suffering with acute Charcot foot and those with vascular problems or an infection that needs a surgical review need to go to the hospital service.

Opening times
We are open Monday to Friday, from 9:00am to 5:00pm. Our telephone lines are open Monday to Friday, from 9:00am to 4:00pm.
Waiting times
We will aim to see those who are triaged as ‘Urgent’ within 10 working days and those that are triaged as ‘routine’ within 8 weeks of referral.
