What does this service provide?
What we do
We are here to help patients with acquired brain injuries, spinal injuries, relapses in multiple sclerosis and people with Parkinson's disease within their home. Rehabilitation is personalised, goal-oriented and high intensity for a period, according to each individual’s need.

Who you'll see at your appointment
Our team is made up of registered professionals and support staff.
During your appointment you may see an occupational therapist, physiotherapist, stroke nurse, speech and language therapist, clinical psychologist or social worker.
Preparing for your appointment
We will contact you to arrange an initial visit within your own home, or by phone or video to establish your priorities and level of need and risk.
We'll provide you with information and resources to help you and your family and there will be at least one face to face visit to check your observations and blood pressure. Alternatively, you can send your blood pressure results into us if you are taking these regularly.
A member of the team will complete a more detailed assessment and work with you to plan therapy and actions which will help you work towards your goals and priorities in a realistic timescale.
Ongoing therapy may be provided by therapy assistants under guidance of registered staff. We actively encourage you to participate and be active in your own rehabilitation and management plans.
We may need to liaise with your GP, consultants, employers, or family members, with your consent.
What happens after my treatment or therapy plan ends?
We will collaborate with you regarding your progress towards your goals and priorities, help you to monitor progress and plan to finish therapy either when you are satisfied with your progress and abilities, or when progress slows or stops and we need to help you with managing your symptoms and abilities longer term.
With your agreement we may refer to less intensive follow up services (NHS) or recommend more social activities or provide resources to suit your preferences, abilities and access or support needs.

How can I access this service?
We accept referrals for people with a confirmed neurological diagnosis and a GP in Brighton and Hove.
Referral is via our referral form by GP, hospital staff or other health related staff. We also accept self-referral.

Additional referral criteria include:
- having suffered a stroke within the last 6 months and have not already received support from the Community Neurological Rehabilitation team
- having acquired a brain injury within last 2 years and have not already received support from the Community Neurological Rehabilitation team
Adults with specific long-term neurological conditions, including:
- Multiple Sclerosis – either new diagnosis or acute, significant change in function
- Parkinson’s Disease – either new diagnosis or acute, significant change in function
- spinal cord injury – recent and requiring neuro rehabilitation for a period of time
- Guillain-Barre Syndrome- recent and requiring neuro rehabilitation for a period of time
- space occupying lesion (non-palliative)
- inflammatory brain injury, for example, meningitis, vasculitis, encephalitis, hydrocephalus.

We are community-based and see the majority of our patients in their homes for their first assessment.
