Midhurst Macmillan Specialist Palliative Care Service

The Midhurst Macmillan Service aims to:

  • Support patients requiring palliative or end of life care to have a good quality of life in their preferred place of care, until the end of their life
  • Increase the number of patients being supported in their preferred place of care and their preferred place of death, and in particular the number of patients supported to die at home
  • Support patients with all diagnoses and not just patients with cancer
  • Provide a service which complies with the NICE Implementing Outcomes Guidance for Supportive and Palliative Care
  • Support independence, choice of care and quality of life, right to the end.
  • Develop a successful and innovative model of care for a home based specialist palliative care service, which reduces the need for traditional bed-based or institutional care
  • Develop an innovative model of close partnership between the NHS, local people and the voluntary sector.

We work in the following healthcare areas:

  • Specialist multidisciplinary assessment
  • Complex pain and symptom management
  • Complex social support.
  • Complex psychological / spiritual issues
  • Information and signposting
  • Volunteer support
  • Bereavement support
  • Provision of supportive and other treatments where agreed with the multi disciplinary team and where appropriate arrangements can be agreed

  • How To Access:

    Referrals will be accepted for any patient over the age of 18 with cancer or any life limiting chronic progressive disease experiencing complex problems that are not responding to routine treatment and therapeutic intervention.

    • Specialist multidisciplinary assessment.
    • Complex pain and symptom management.
    • Complex social support.
    • Complex psychological / spiritual issues.
    • Information and signposting.
    • Volunteer support.
    • Bereavement support.
    • Provision of supportive and other treatments where agreed with the multi disciplinary team and where appropriate arrangements can be agreed.
    • Provision of written information

    Patients registered with the General Practitioners from the following list of Practices may be referred to the service:
