The role of the Community IV Therapy Service is to support and facilitate early hospital discharge and avoid admissions of patients to acute trusts through delivery of IV therapies at home. The IV Team supports and trains community nursing teams within Sussex Community NHS Foundation Trust to enable the safe administration of IV medications to patients within their care. The development of IV clinics, in some areas of the Trust, is supporting a greater number of patients to remain within the community and for us to deliver care closer to home.
The IV Therapy Service works with patients in the West Sussex area of Sussex Community NHS Foundation Trust.
The teams are based in four areas of West Sussex:
Coastal South: Worthing, including Southwick, Shoreham, Lancing, Worthing, Littlehampton, Steyning, Storrington and Henfield
Coastal West: Chichester, including Arundel, Bognor Regis, Selsey, Chichester, Pulborough, Petworth and Midhurst
North: Crawley, including Horsham, Crawley, Southwater, Billingshurst, Rudgwick and Horley (only patients registered with a West Sussex GP)
Mid Sussex: Haywards Heath, including Burgess Hill, Cuckfield, Haywards Heath, Crawley Down and East Grinstead
Patients must be registered with a West Sussex GP.
