Intermediate Care (ICT) Mid Sussex
Intermediate Care Mid Sussex has two components.
Intermediate Care Community Team: The team is nurse/therapy lead and is based at the Kleinwort Centre. It provides short term rehabilitation for patients aged over 18, with the aim of preventing unnecessary hospital admissions and supporting timely discharges from the beds at the Kleinwort Centre and acute hospitals. Patients continue to be supported by their own GP.
Inpatient Unit: A 31-bedded unit providing short term rehabilitation for patients aged over 18, with the aim of preventing unnecessary hospital admissions and supporting timely discharges from the acute hospitals.

- How To Access:
Referrals are made through a single point of access (OneCall) which operates 24 hours a day, 7 days a week.
Patients are referred for rehabilitation for:- an acute worsening in their health such as infection
- an acute worsening of their long term condition such as heart failure or Parkinsons Disease
- an acute hospital admission for illness or surgery
- a fall
- an adjustment of complex medication
