Our 14 intermediate care beds at Uckfield Community Hospital provide short term rehabilitation for patients aged over 18, with the aim of preventing unnecessary hospital admissions and supporting timely discharges from the acute hospitals.
The service is nurse/therapy led. On admission to the service, we complete a thorough assessment with each patient and a rehabilitation care plan is negotiated with them. Patient's medical needs are met by local GPs.
