Who we are
We are a team of specialist practitioners providing support at home to patients with Parkinson’s disease.
What we do
Our service aims to improve the quality of life for people diagnosed with Parkinson’s disease and associated conditions and provide support to their families and carers through advice and education.
Our service includes:
- reviewing medication
- providing lifestyle advice
- emotional support with problem solving
- helping patients access additional support and advice regarding symptom management
- providing advice, information and support through telephone reviews and referrals to multidisciplinary teams / agencies where needed.

Who you'll see at your appointment
During your appointment you will typically meet one-to-one with one of our Parkinson's disease specialist practitioners from your local team.
Where your appointment will be
We visit patients in their homes. However, clinic appointments are available at Horsham Hospital for patients registered with a Horsham, Cowfold or Rudgwick GP.
How can I access this service?
To access our service, patients must have a diagnosis of Parkinsonism, that is Idiopathic Parkinson’s, Multi System Atrophy (MSA), Progressive Supranuclear Palsy (PSP), Corticobasal Degeneration (CBD) or Vascular Parkinsonism, and have a consultant neurologist.
We accept referrals from any healthcare professional, via our One Call team on 01293 228 311.
Where can I access this service?
Please note that each of our local teams work varying patterns and hours and we are often out of the office in clinic or on home visits, so may not be able to answer your call straight away. Please leave a message on our answerphone and we will get back to you as soon as possible.
Waiting times
Your referral will be triaged and if you are accepted into the service, you will receive a contact letter.
