Who we are
We are a team of Motor Neurone Disease (MND) specialist nurses and practitioners providing support to patients diagnosed with MND and their families.
What we do
We aim to improve the quality of life for people diagnosed with MND and their families. We do this through providing support and advice in people’s homes and providing a continuous point of contact.
Our services include:
- regular monitoring, assessment and intervention
- working with and listening to the needs of patients with MND and their carers
- symptom management
- working with and referring to other health professionals to provide a multi-disciplinary approach to care
- advance care planning to help patients with MND and their carers talk through options and preferences for their future care

We offer home visits, clinic appointments and telephone reviews.

During your appointment you will typically meet one-to-one with one of our MND specialist nurses or practitioners from your local team.
How can I access this service?
We operate an open referral system for patients who live in Brighton and Hove and West Sussex and are registered with a GP. We will need confirmation of your diagnosis from your GP or consultant before your first assessment.

We aim to assess all our referrals within four weeks.

Each local team works varying patterns and hours and we are often out of the office in clinic or on home visits so may not be able to answer your call straight away. Please leave a message on our answerphone and we will get back to you as soon as possible.
