The 28 beds on Viking Ward provide short term rehabilitation for patients aged over 18, with the aim of preventing unnecessary hospital admissions and supporting timely discharges from the acute hospitals. On admission to the service, we complete a thorough assessment with each patient and a care plan and therapy goals are negotiated with them. Patient’s medical needs are met by a ward doctor supported by our Advanced Nurse Practitioners Monday to Friday with out of hours cover provided by IC24.

Referrals are accepted from all health and social care professional, including GPs, community nurses and matrons, social workers, hospital consultants and nurses, paramedics and emergency care practitioners. Community referrals are made via the Clinical Assessment Unit (CAU) pathway.

Visiting outside of these times can be negotiated with the nurse in charge if appropriate.
