Inpatient Unit - Horizon Unit, Horsham Hospital

The 38 beds on Horizon Unit short term rehabilitation for patients aged over 18, with the aim of preventing unnecessary hospital admissions, supporting timely discharges from the acute hospitals and providing palliative care. On admission to the service, we complete a thorough assessment with each patient and a care plan is negotiated with them. Patient’s medical needs are met by a ward doctor Monday to Friday with out of hours cover provided by IC24.



How To Access

Referrals are accepted from all health and social care professional, including GPs, community nurses and matrons, social workers, hospital consultants and nurses, paramedics and emergency care practitioners.
Referrals are made through a single point of access (OneCall) which operates 24 hours a day, 7 days a week.
Patients are referred for rehabilitation following:

  • an acute worsening in their health such as infection

  • an acute hospital admission for illness or surgery

  • a fall

  • an adjustment of complex medication

  • palliative care

The beds are open 24 hours per day 365 days per year.
