Community Rapid Response Service (Brighton & Hove)

We are a multidisciplinary service, provide a rapid assessment to patients / clients age 18 above. We work Monday to Sunday and cover from 8am-8pm including public holidays. We aim to make contact and/or assess a patient within 2 hours of receiving a referral, and will support them for up to 72 hours through the crisis period.

  • CRRS will inform both patient and next of kin (if needed) and their GP on discharge from the service.
  • We will also inform you whether you have been referred on to a different service.

The service is mainly for admission avoidance that is if someone has been seen by their GP at home and does not need to go into hospital but their needs can be met in their own homes; then your GP will refer to our service.

We visit patients in their own homes / hospital mainly the Accident & Emergency department. We also see patients in care and nursing homes.

We look after patients with any types of infections e.g. chest infection, patients who have had falls and if they are not managing at home.

We provide assessment and support you with both health and personal care needs, including daily living tasks. We will help you with night sitters or night care if needed.
Our service is for people who have a Brighton and Hove GP only. You may live in Southwick but if your GP is in Brighton and Hove, then you can be referred to the service.

Our service covers three areas of Brighton and Hove, East, Central and West. East covers Woodingdean, Rottingdean, Saltdean, Hollingdean, Moulsecoomb, Coldean and Whitehawk. Central area covers Withdean, Patcham, Preston Park, Varndean and West covers Hove, Hangleton, Portslade and Mile Oak.

How To Access

The public cannot make direct referrals to our service. Referrals are made via GPs to avoid hospital admission.

Referrals are accepted from GPs via the Professional Support Line on 0300 130 3045 which covers the community.

Patients can also be referred to our service from departments in any hospital including A&E, AMU, ACU, CDU and all wards via Single Point of Contact on 01273 242117.
