Responsive Services West Sussex
We are a multi-disciplinary team which includes Nurses, Physiotherapists, Occupational Therapists and Rehab and Nursing Assistants. We provide person-centred, time limited, health support to people in their home environments, and if longer term needs are identified, we will signpost patients on to the relevant services.
We are a West Sussex-wide service, working alongside West Sussex GP Practices.

Referrals are accepted from Health and Social Care providers.
Responsive Services are not able to accept referrals directly from patients or relatives.

Once a referral is accepted, the patient/family will be provided with the contact details of the relevant geographical team.
