Respiratory Service for Chronic Obstructive Pulmonary Disease (COPD) West Sussex

The Community Respiratory Service offers care to the patients with Chronic Respiratory condition (COPD) in the community. We offer care to include making sure the patients are on the right treatment, Oxygen assessments, we provide Pulmonary Rehabilitation and also support the Breathe Easy Groups in the localities.

The team of the Specialist Nurses and Physiotherapists provide care both in home and clinical settings with the aim of improving quality of life and planning care to suit individual needs. We also monitor the patients electronically at home via Telehealth technology.

  • How To Access:
    We take referrals from the GP surgeries, the Secondary care hospitals and any other health professionals who identify the need for specialist care. Patients known to our service can refer themselves when care is needed as necessary. Referral forms are available for SCT staff on SCT staff intranet where referral criteria for Nurses/Pulmonary Rehabilitation are clearly outlined.

    We are a team consisting of specialist nurses, physiotherapists, health care assistants and administrators, providing patient support around optimisation of treatment, pulmonary rehabilitation and oxygen therapy. We are also able to provide information on where to buy or rent nebuliser compressors.
    We do not accept self-referrals. Please contact your GP or healthcare professional if you wish to discuss your respiratory condition.

    Although we are located in the hospital, we do not hold any clinics routinely and any appointments will be scheduled and clinic room details clearly explained. Otherwise the hospitals have good community bus links and disabled parking available. Parking is limited and we recommend that you take public transport where possible.
