Prevention Assessment Teams West Sussex (PATs)

We're multi-agency, multidisciplinary teams delivering a preventive service across West Sussex. The team includes health advisors (qualified health professionals); social care workers and support workers from the voluntary sector who can advise over the telephone or come and see you in your own home. Our objective is to improve quality of life and the promotion of health and wellbeing.

Our service is for adults and older people who might not be eligible for other statutory services and who have unmet physical, psychological, social, functional and/or environmental needs.

We provide information provision, education, advice and support, to enable independence and self-care, and undertake proactive interventions and problem solving. This reduces the risk of the need for reactive, more intrusive and costly interventions at a later date.

We also provide NHS health checks in outreach settings in the community as well as health promotion sessions to community groups across the county. See more details

We can help by:

  • Advising you how to keep healthy and manage health issues
  • Link you with agencies that can maximise your income and help you remain independent through practical tasks such as shopping, cleaning and gardening
  • Put you in touch with local social groups and activities
  • Help you keep your home safe, secure and warm
  • Let you know what transport services are available locally

  • How To Access:
    Any referrals must be made with client consent. They can be self referrals or from other health and social care staff, other statutory and voluntary sector partners or the well-being teams. Referrals can be made by telephone, by fax, by post or by secure e-mail.

    Exclusion criteria:
    • Adults and older people who are residents of residential & nursing homes
    • Adults and older people who are terminally ill
    • Those diagnosed with mental health problems that are currently experiencing an acute phase and/or are being supported by other community services
    • Adults and older people with multiple complex chronic disease management needs
    • Adults and older people with multiple complex social care needs
    • Children and young people under 18 years
