Parkinson's Disease Specialist Practitioner
The Parkinson's Disease Nurse Specialist Service aims to improve the quality of life for people diagnosed with Parkinson's (including associated conditions), and their families. This is done by providing support, advice and education about Parkinson's Disease in people's homes. For example:
- Clinical monitoring and medication adjustment.
- A continuous point of contact.
- Reliable information about clinical matters and any other issues that concern people with Parkinson's and their carers.
- Liaison with other professionals in hospital, nursing homes and GPs for example.

- How To Access:
We operate an open referral system. For new clients we will need confirmation of diagnosis from your GP or Consultant before assessment.
The following criteria must be met:
- Have a diagnosis of Parkinson's disease (or associated condition e.g. Progressive Supranuclear Palsy/Multiple System Atrophy/Corticobasal Degeneration)
- Unable to attend hospital based neurology/elderly medicine outpatient appointments OR will have complex needs requiring support in the community to avoid hospital readmissions.
- Must be over 18 years of age.
- Registered with a West Sussex GP
