Palliative Care Team (Brighton & Hove)
We are a specialist team of nurses, doctors, occupational therapists and social workers, who aim to promote high quality holistic care for people living with and dying from life limiting illnesses. The service we offer includes assessment of physical, emotional, psychological and spiritual needs, as well as pain relief and the management of other symptoms. We support relatives and carers and provide expert advice to other health care professionals.

- How To Access:
Patients aged 18yrs+ with locally advanced / metastatic cancer with complex physical and / or emotional problems related to their disease and patients aged 18yrs+ with other life limiting disease who have complex end of life needs for:
- Symptom management
- Decision making and/or advance care planning
- Psychosocial/ emotional support
- Hands on end of life care support (Hospice@Home)
- Respite support (Hospice@Home)
- Signposting to other relevant support services
It is expected that these patients will usually be on a palliative care register such as that used as part of the Gold Standards Framework (GSF). Reference to the GSF Prognostic Indicator guidance may help determine who these patients might be.

As we offer a specialist service we are unable to accept self referrals unless the person has been known to the team previously. We accept referrals from health care professionals in the wider health economy. Please ensure the patient's GP has been informed.

An on call consultant is available for telephone support for health care professionals 24hrs. - How to find us:We are mainly a visiting service, but offer assessments at our out-patient clinic based at the hospice. There is a small car park and street parking available.
