What does this service provide?
Who we are
We are a team of nurses and healthcare assistants who aim to prevent hospital admissions and provide continuation of urgent clinical care.
We have 4 teams covering Arun, Adur, Chichester and Crawley during the evening (6:00pm–11:00pm), a Coastal Overnight Team and a Central Overnight Nursing Team.
What we do
We provide urgent out of hours nursing intervention and support in the community throughout West Sussex. We provide end of life care and symptom management, catheter care, urgent wound care, urgent diabetes management and admission avoidance with deteriorating patients when appropriate.
In addition, we provide a night support service to frail, end of life patients and for those who are acutely unwell and admission avoidance is required.
Appointment and visiting information
Due to the nature of urgent care, we do not offer timed appointments.
We aim to achieve visits within two hours of your call between 6:00pm and 8:00am.
How can I access this service?
Patients need to be registered with a West Sussex GP.
Referrals are allocated to the relevant team by OneCall or ECHO, who can be contacted on 0300 3737 111.
