Looked After Children Nurse Specialist Team
A team of specialist nurses who are committed to improving the health outcomes for children and young people who are looked after. This specialist, county-wide team strive to deliver a supportive, informative and highly effective service to address and meet the health needs of children and young people who are looked after
We achieve this by:
- Undertaking and overseeing nurse led health assessments which are bi-annual for children under 5 years old and annually for children aged of 5- 18 years
- Providing all children and young people who are looked after with a direct contact number of their specialist nurse.
- Delivering effective health promotion to vulnerable young people
- Adopting a holistic approach to each individuals needs
- Supporting and advising foster carers to improve health needs of looked after children and young people
- Provide all 16-17 year olds with a summary of their health history
- Attend meetings pertinent to the wellbeing of children and young people (strategy, CLA reviews)
- Proactive engagement with residential care home staff and residents, and young people in supported living accommodation.
- Attendance at foster panel
- Attendance at Therapeutic Access Panel (TAP)
- Oversight of all West Sussex children and young people placed out of West Sussex , coordinating health assessments, and liaising with providers in placement area to ensure continuity of healthcare

Foster families, Social Workers and other professionals can access the services of the Looked After Children Nurse Specialist Team via the following routes:
- Telephone
- Meetings
- Via other health professionals such as Doctors, School Nurses and Health Visitors
- The Virtual School
No referral is necessary. The Team are notified of all children placed in care within West Sussex. This begins the process of health needs evaluation.
The Looked After Children Specialist Nursing Team is not an emergency service and does not have an 'out of hours' service.
