The Community IV Therapy Service supports and facilitates early hospital discharges by providing and supporting the delivery of IV therapy within the primary care setting. The aim of the IV service is also to reduce hospital admissions by developing and delivering community IV pathways including the GP initiated Cellulitis pathway within the Brighton and Hove area. This provides patients with an alternative care setting and improves the patients'
experience.
Our service is for people who have a Brighton and Hove GP only. You may live in Southwick but if your GP is in Brighton and Hove, then you can be referred to the service.
Our service covers three areas of Brighton and Hove, East, Central and West. East covers Woodingdean, Rottingdean, Saltdean, Hollingdean, Moulsecoomb, Coldean and Whitehawk. Central area covers Withdean, Patcham, Preston Park, Varndean and West covers Hove, Hangleton, Portslade and Mile Oak.
The public cannot make direct referrals to our service. Referrals are made via GPs to avoid hospital admission and from hospital staff discharging patient into our service for IV therapy.
Referrals are accepted from GPs via Professional Support Line on 0300 130 3045 which covers the community.
Patients can also be referred to our service from departments in any hospital including A&E, AMU, ACU, CDU and all wards via Single Point of Contact on 01273 242117.
