Integrated Response Team
Support for care homes in North West Sussex
The team provides advice, training and information for care homes that provide care to older people and people with learning disabilities. The aim is to support care homes to deliver high standards of care. There is close liaison with GP's, WSCC Social Services, South East Coast Ambulance Service, community nurses and community based services.
We work with care homes within Crawley, Horsham, East Grinstead, Haywards Heath and Burgess Hill. The team consists of nurses, a dementia nurse specialist a dietitian and administrator. We work closely with WSCC contracts, safeguarding and quality teams within the Clinical Commissioning Groups.

- How To Access:Access is through contacting the team directly. This can be done through: social services, care homes or by any healthcare professional who has concerns about care home residents' welfare e.g. number of ambulance call outs or levels of requested interventions.
