Inpatient Unit - Horizon Unit, Horsham Hospital

Horizon Unit has 38 beds providing short term rehabilitation for individuals over 18 years of age.

People are referred for rehabilitation following an acute worsening in their health such as infection, an acute hospital admission for illness or surgery, a fall, an adjustment of complex medication or palliative care.

The ward provides a multidisciplinary service that supports individuals with the aim of regaining independence as much as possible.

On admission, a thorough individualised assessment is undertaken and a person centered approach is adopted to develop an achievable, motivational care plan in partnership between the multidisciplinary team, the patient and their relatives and carers. The care plan will focus on individual strengths and therapy goals are negotiated in collaboration with the aim of increasing the individual's knowledge, resilience and confidence in regaining independence.

We have a dedicated team of physiotherapists and occupational therapists to provide support and promote independence with mobility and personal care skills. They assess the individual's needs in the community and can refer them on to the right health and social care professionals for continuity of care. 

Patient's medical needs are managed by a ward doctor from Monday to Friday, with support from an Advanced Nurse Practitioner with out of hours cover provided by IC24 out of hours GP service.

Additional beds

Horizon Unit will temporarily provide additional beds throughout the winter. Six beds opened on 1 December 2022, with a further six to open in the coming weeks.

    Hospital referrals: The hospital discharge team refer individuals to the patient flow center and an available rehabilitation bed is allocated depending on ward capacity

    Community referrals:-

    • Out of hour referrals are made through liaisons between healthcare professionals and the Clinical Assessment Unit (CAU) via an agreed pathway
    • During working hours, referrals are made directly between the GP and the Doctor based on Horizon unit

  • Opening Hours:

    Horizon is a 24 hour service.

    Visiting times are from 11am to 8pm with reduced visiting at mealtimes.

    Visiting times can be negotiated if the patient requires support from their own carer due to cognitive impairment or other needs.
