Heart Failure Service (Brighton & Hove)

We are a community heart failure (HF) team working across Brighton & Hove. We see patients who are over 18 years of age with a Brighton & Hove GP with a confirmed diagnosis of heart failure of any cause by echocardiogram.

We visit patients in their private residence, nursing or care home, GP practice, HF clinic and if not suitable in the community referred to best place of care working towards a target of reducing hospital admissions. We provide the patient with our contact details and written and verbal information around the management of heart failure; we give education around self-management and recognising early signs or symptoms of heart failure and when to contact us for telephone advice or review.

We work as per NICE guidance for Heart Failure (2010) and aim to optimise as indicated preventative medications for heart failure such as ACE inhibitors and Beta-blockers. We work closely with the GP in managing patients with heart failure. We provide a personalised care plan for heart failure management and share care with the GP and other community services.

We work collaboratively with the consultant cardiology/medical teams and heart failure nursing service at the Royal Sussex County Hospital and meet regularly to discuss more complex cases ensuring safe and effective care.

    Patients are referred to the community heart failure service by secondary care (Hospital), GPs or other healthcare professionals.
    Our referral criteria is:

    • Patients over 18 years of age
    • With a Brighton & Hove GP
    • With a confirmed diagnosis of heart failure of any cause by echocardiogram

    We see patients with all types of chronic heart failure. We require a completed referral form sent to us by fax, post or email. It should include the reason for referral and if the referral is Urgent (at risk of admission within the next 48 hours) or Non-urgent. We aim to all patients within two weeks.

    In an emergency you must call 999 or your GP. We are not an emergency or rapid service and see all patients on an appointment basis. If a patient is at risk of admission we can see a patient within 48 hours of the referral being accepted.
