ECHO - End of life care hub

What is echo?

echo improves the coordination and delivery of end of life care across Coastal West Sussex by linking key services via a 24/7 telephone coordination hub staffed by trained nurses.

What are the benefits of echo?

If you have a terminal illness, or are approaching the end of your life, echo acts as a single point of contact to offer advice and support at what can often be a difficult time for you and your family. We will record your views and wishes within a confidential personalised care plan. This care plan will be shared only with health care professionals involved in your care to help them understand the choices you've made.
