Diabetes Care for You

Diabetes Care for You is a service for adults living with Type 1 and Type 2 diabetes and registered with a GP practice in the Brighton and Hove and High Weald, Lewes and Havens area. This service helps people to live well with diabetes by offering advice, support and help with diabetes management and meeting personalised goals.

The diabetes team

The team brings together consultants, diabetes specialist nurses, dietitians, podiatrists and psychologists, all of whom specialise in care for people with diabetes. The team works with you to explore your health needs, and to help you to understand and manage your diabetes in the best way possible.

The service has close links with hospital based diabetes services to ensure that if needed, you will have access to all of the services and further specialist clinicians your condition requires.

What we offer

  • Annual review and ongoing support and care for people with Type 1 diabetes, including close working with paediatric teams for young people moving into adult services.
  • Support and treatment for people that need extra help to manage their diabetes, including insulin and GLP-1 initiation for people with Type 2 diabetes.
  • Group diabetes education sessions.
  • Emotional and psychological care to support diabetes self-management and general wellbeing.
  • Specialised support for women with diabetes that are trying to conceive.

We also provide advice and education to GPs and practice nurses to help them provide extra support for people with diabetes in primary care.

    Referrals are usually made by your GP to this service, but the service can also be accessed through a referral from a hospital team or other healthcare professional.

    If you are supported by the service and have a query about your care or are not being supported by the service but would like to know more, you can contact the team through our Diabetes Admin Hub on 0300 373 1886 and they will assist you with your query.

    If you need urgent help

    Diabetes Care for You is not an urgent or emergency service. If you need urgent help:

    • In an emergency, go to A&E or dial 999
    • Contact your GP immediately or call NHS 111 if your surgery is closed
  • Opening Hours:

    The Diabetes Care For You Service operates Monday-Friday 9am to 5pm however please note the phone lines differ and are open Monday-Friday 9am to 4pm.
