Who we are
We are a specialist community-based neurological team comprising of registered and unregistered staff who visit people in their own homes.
What we do
We see patients with a confirmed neurological diagnosis who require support from a multi-disciplinary team. This includes conditions such as Parkinson's Disease, Multiple Sclerosis, Motor Neurone Disease, stroke, brain injury as well as many others.

Who you'll see at your appointment
Our team is made up of physiotherapists, occupational therapists, a stroke nurse, speech and language therapists, therapy practitioners and rehabilitation assistants.
Preparing for your appointment
We will provide an initial assessment in your home or by telephone to establish your priorities and level of need and risk. There is usually a waiting time for therapy which will be discussed at your first appointment. Together we work towards your personal rehabilitation goals to help you improve your abilities and function in day-to-day life. We will work closely with you and your relatives and carers and give you things to work on in-between visits.
There will be at least one face-to-face visit to check your observations and blood pressure, or you can send these to us if you are already taking these.
Our specialists will provide a more detailed rehabilitation assessment, for example, one of our physiotherapists, speech therapists or occupational therapists. They will plan therapy and create actions to help you move towards your goals and priorities in a realistic timescale.
Ongoing therapy and practice may be provided by therapy assistants under guidance of registered staff. We will encourage you to participate and be active in your own rehabilitation and management plans. We'll provide education and advice regarding your condition and any actions you can take to improve long term outcomes of health and wellbeing. With your consent, we may need to liaise with your GP, consultants, employers, and family members.
What happens after my treatment or therapy plan ends?
You will be given advice to follow after your therapy ends. If you have a long-term neurological condition, you can usually self-refer yourself back to the team.
How can I access this service?
Our service has specific referral criteria. Please discuss potential referrals via the phone or email – details are available in ‘who to contact for more information’. For new patients we will need confirmation of diagnosis from a GP or consultant before we can assess you. All patients need to have a registered GP in West Sussex, within the locality of Adur, Worthing and Chanctonbury.
