Children and Young People's Community Nursing Service

Sussex Community NHS Foundation Trust is an award-winning lead provider of children and young people’s community nursing services across West Sussex and Brighton and Hove. 

We support children and young people with a clinical nursing need aged 0 to 18 years who have:

  • acute, short-term conditions
  • long term conditions
  • complex health needs and/or disability
  • life limiting and life-threatening conditions

 Our service includes:

  • Four locality Children and Young People’s Community Nursing teams in the North Crawley, South Lancing, West Chichester and Brighton and Hove
  • Special School Nursing
  • Specialist Children and Young People’s Continence Team

Our team can:

  • offer specialist care and advice
  • plan visiting schedules in collaboration with you and your family, arranging mutually convenient appointments within a two-hour window
  • provide a named nurse for you and your family, with clear contact details if you need to get in touch

We aim to:

  • provide your child with high quality nursing care at home
  • enable the safe and effective discharge of your child from hospital
  • prevent their admission or re-admission to hospital
  • support your child’s overall health and wellbeing
  • enhance quality of life for your children and the whole family
  • provide or help you access specialist paediatric equipment that allows your child to be cared for at home
  • provide advice to children and their families to help them to manage their nursing care

 Our skilled nurses provide care and support for a range of illnesses and conditions including:

  • cancer (in Brighton and Hove this is provided by Royal Alexandra Children’s Hospital)
  • heart problems
  • epilepsy
  • oxygen dependency
  • rheumatology (issues with joints, muscles and ligaments)
  • multiple complex needs
  • airway problems, including tracheostomy
  • enteral feeding (the feeding of liquid through a tube direct to the stomach)
  • post-transplant care
  • wound care
  • complex bladder and bowel management
  • renal (kidney) and hepatic (liver) problems
  • long term ventilation and oral, nasal or nasopharyngeal suction needs
  • short term intravenous therapy
  • treatments by regular injection

  • How To Access:

    The service will receive referrals from registered healthcare professionals from paediatric services within the acute/tertiary services, general practice, specialist nursing teams and children’s therapy teams.

    All referrals should be made electronically. Referrers can contact the service for advice on referrals via telephone. Contact details for local area teams are below.

    Please complete the main referral form and any additional accompanying form for nasogastric (NG)intravenous (IV) and wound referral.

  • Opening Hours:

    Our service is open from Monday to Friday, 8am to 6pm. Weekends and Bank Holidays 8.30am to 1.15pm. 

    The Brighton and Hove team is not currently offering a weekend and Bank Holiday service. 

    End of Life Care: 24-hour standby care, in partnership with Snowdrop at Home care and Chestnut Tree House.
