Who we are

We are a team of therapists and support staff who provide assessment and therapy to patients suffering with any neurological conditions.

What we do

We are here to help patients suffering with any neurological conditions. This includes people who have been recently discharged from hospital following an acute neurological event such as a stroke, brain injury or spinal cord injury, alongside people with a long-term neurological condition such as multiple sclerosis, Parkinson’s, and motor neurone disease.

We offer a variety of options, including home visits, outpatient physiotherapy and virtual online therapy. 

Who you'll see at your appointment

Our team is made up of registered professionals and support staff.

During your appointment you may see an occupational therapist, physiotherapist, stroke nurse, speech and language therapist, clinical psychologist or social worker.

Preparing for your appointment

You will receive a full assessment from one of our therapists or rehabilitation technicians, which will help identify any issues you are currently experiencing and set some goals for you to work towards with support from the team.

You will also be set a treatment plan which you will work on by yourself and/or with support of the therapist or support staff. You may be signposted to other teams and services which may also be able to support you.

What happens after my treatment or therapy plan ends?

You will be given advice to follow after your therapy ends. If you have a long-term neurological condition, you can usually self-refer yourself back to the team.


How can I access this service?

Initially, you will need to be referred to the team via a healthcare professional - usually a therapist from the hospital that discharged you or via a consultant, GP, specialist practitioner or another health professional involved in your care.

To be referred, you must be registered with a West Sussex GP, or:

  • patients registered with Woodlands and Clerklands (Horley) surgeries
  • patients registered with a Billingshurst GP if they live in our area

Additional referral criteria include:

  • having suffered a stroke within the last 6 months and have not already received support from the Community Neurological Rehabilitation team
  • having acquired a brain injury within last 2 years and have not already received support from the Community Neurological Rehabilitation team

Adults with specific long-term neurological conditions, including:

  • Multiple Sclerosis – either new diagnosis or acute, significant change in function
  • Parkinson’s Disease – either new diagnosis or acute, significant change in function
  • spinal cord injury – recent and requiring neuro rehabilitation for a period of time
  • Guillain-Barre Syndrome- recent and requiring neuro rehabilitation for a period of time
  • space occupying lesion (non-palliative)
  • inflammatory brain injury, for example, meningitis, vasculitis, encephalitis, hydrocephalus.
  • self-referrals for patients with a long-term condition previously known to the team if they have been seen for therapy in the last 12 months by the Community Neurological Rehabilitation Team. 
  • single discipline referrals are not accepted for dietetics, psychology, or social work – there must also be another therapy required

Where can I access this service?

We are community-based and see people in their homes but we do also hold outpatient physiotherapy clinics at Horsham Hospital and Saxonbrook Medical Centre, Crawley. If attending these, you will be sent a map with your appointment letter.

What to do when you arrive

Follow the yellow-coloured signs and our Neuro Physiotherapist will meet you in the neuro physio waiting area. 

If you are travelling to the hospital by car, please allow extra time for parking as the hospital car park and surrounding roads get very busy.

Opening hours

We are open Monday to Friday, from 08:30am to 4:30pm. We are closed on weekends and bank holidays.
