What does this service provide?
Who we are
We are district nurses, community nurses, healthcare assistants, nursing associates and administrators who work in local teams across East Sussex.
What we do
We actively promote health and wellbeing in the community, and where possible prevent admission to hospital for patients in their own homes.
How can I access this service?
To access our community service you must be housebound, which means you are unable to leave your home, or live in a Care Home. You must be an adult aged over 18 years and be registered with a Brighton and Hove GP or a GP in the High Weald, Lewes and the Havens area.
If you meet the above criteria, you can ask your GP or other health care professional to refer you.
We are not an urgent response service and do not respond to urgent referrals.
Where can I access this service?
Opening times
All Brighton and Hove Nursing Teams work from 8:00am to 8:00pm and High Weald Lewes and Havens Nursing Teams work from 8:00am to 10:00pm.
