What does this service provide?

Who we are

We are a team of healthcare professionals in West Sussex who work closely as part of a multi-disciplinary team, which is made up nurses, Care Home Matrons, health care support workers, physiotherapists, occupational therapists, social workers, mental health practitioners, psychological therapists and organisations which provide voluntary services.

What we do

The Primary Care Network Community team has three functions. It provides community nursing, Proactive Care and promotes Enhanced Health in Care Homes. If you are housebound or in a Care Home and unable to get to your GP practice or any other healthcare setting, our community teams will visit you at home and provide a personalised assessment of your health needs with an agreed plan of care to support you. We have a proactive approach which aims to help you stay as healthy as possible and to live independently in the community for as long as possible. If you have a long-term condition (chronic illness) our Proactive Care team will help you to plan for the future, enabling you to stay as well and independent as possible for as long as possible. If you are a resident in a Care Home setting our Care Home Matron will work with you and the Care Home to ensure you can access the same services that would be available to you if you still lived in your own home.

We offer face to face visits at home, and telephone appointments.

Who you'll see at your home visit

Who you see at your home visit depends on your individual needs. Once we've received your referral, the appropriate health care professional will visit you at home.

Preparing for your appointment

Visits will take place at your home, and you can expect to be treated with kindness and compassion and to a professional standard of care. We will plan care that meets your needs to support you to meet your goals. We will regularly assess and review your ongoing care and agree a treatment plan with you. We will always confirm our identity before we come into your home.

In return, we ask that you are available when we visit and provide a safe working environment for our staff which includes hand washing facilities. Please inform us of any risks which may be present whilst we are in your home.

We ask that you follow the course of treatment recommended by your healthcare professional and speak to us if you are finding this difficult. There is zero tolerance of abusive behaviour, and we have the right to end our visit should we feel threatened or at risk.

We also ask that if you have any pets such as a dog, that you secure them away whilst we are visiting. Please do not smoke prior to or during our visit.

What happens after my treatment or therapy plan ends?

We will continue visiting you until your care is complete, or until you are able to self-manage, or the care can be provided within another healthcare setting. We will transfer your care back to your GP practice if you no longer meet the criteria as a housebound patient.

You may receive a request from time to time to complete a Friends and Family Test questionnaire. We would be grateful if you would take the time to complete these so we can find out about your experience with our service and if necessary, we can look to make improvements as a result of any feedback received.


How can I access this service?

To access our community service you must be housebound, which means you are unable to leave your home, or, in a Care Home. You must be an adult aged over 18 years and be registered with a West Sussex GP.

If you meet the above criteria, you can ask your GP or other health care professional to refer you via OneCall into our community service. For Proactive care the referral process is the same. Once you are under our services, you can liaise directly with the teams via one-call.

Opening hours

Our service runs 7 days a week, from 8:00am to 6:00pm. However, you can contact OneCall 24 hours a day and they will direct you to the most appropriate services depending on your needs.
