Primary Care Network Community Teams (Brighton & Hove)

Formerly known as District/Community Nursing.

We work closely with your GP to help you get the care you need. We'll:

  • Review your health condition, assess your needs, agree with you a health action plan and help you care for yourself.
  • Provide care to you wherever you live.
  • Ensure you get the health services you need, such as education and advice, medicines and continence services. For some services you'll need to go to your GP surgery - if you can.
  • Work with all specialist teams to ensure you get the right care, from the right people, with the right skills, at the right time.
  • Work with social services to ensure care plans and support packages meet your needs.

We provide care to:

  • People aged 16 and over.
  • Who are registered with a GP in Brighton & Hove.
  • Who have a long term condition or who are frail or elderly and housebound.
  • A long term condition is a health a condition that cannot be cured but can be managed with therapy and/or medication.

Teams provide:

  • Advanced physical assessment skills
  • Independent prescribing and case management.
  • Care co-ordination.
  • Nursing staff.
  • Allied Health Professionals.
  • Community medication review pharmacist.
  • Generic support workforce.

    Referrals can be via a GP, other health professional or by self referral to Primary Care Network Community Teams.
