Caravelle Ward is a 28 bedded ward that provides short term rehabilitation for individuals over 18 years of age. Individuals are referred for rehabilitation following an acute worsening in their health such as infection, an acute hospital admission following illness or surgery, a fall or an adjustment of complex medication. The ward provides a Multidisciplinary service that supports individuals with the aim of regaining independence as much as possible. The service objectives are to prevent unnecessary hospital admissions and to support timely discharges from the acute hospitals. On admission to the service, a thorough individualised assessment is undertaken and a person centered approach is adopted to develop an achievable, motivational care plan in partnership between the Multidisciplinary Team, patients, relatives and carers. The care plan will focus on individual strengths and therapy goals are negotiated with the aim of increasing individual’s knowledge, resilience and confidence in regaining independence. Individuals medical needs are managed by a ward doctor supported by an Advanced Nurse Practitioner Monday to Friday with out of hours cover provided by IC24 out of hours GP service.
Referrals to Caravelle Ward are made via acute hospital trusts and CAU at Crawley Hospital by means of agreed pathways. During weekdays, SCFT nurse assessors based at the acute hospitals liaise with the patient flow center and an available rehabilitation bed is allocated depending on ward capacity.

Visiting Times From 11:00 -20:00 with reduced visiting at mealtimes.
Visiting times can be negotiated if the patient requires support from their own carer due to cognitive impairment or other needs.
