Diabetes Education and Self Management for Ongoing and Newly Diagnosed (Desmond) Courses

Following your diagnosis of Diabetes your practice nurse can refer you to attend the Diabetes Education session known as DESMOND. The purpose of a Desmond course is to help newly diagnosed Type 2 diabetes patients take control and manage their diabetes proactively. DESMOND (Diabetes Education and Self Management for Ongoing and Newly Diagnosed) is a nationally accredited course. This is a one-day course, but some courses are offered in two half-day (equivalent) sessions.

You are invited to join a small group of up to ten people with Type 2 diabetes on a DESMOND programme led by two fully trained educators. You will be given up to date information, and learn practical skills to manage your diabetes. An opportunity will be provided to discuss and explore factors relating to diabetes such as food choices, activity and medication. You will also be able to meet and talk to others in the same situation.

Throughout Mid-Sussex, Crawley & Horsham we are proud to offer this FREE education programme for people who have been diagnosed with Type 2 Diabetes and after running these courses in the Horsham and Crawley areas for over three years the participants have found the course very informative, enjoyable and beneficial in managing their diabetes.

    Referral Criteria:
    The course is designed for Patients Newly diagnosed with Type 2 Diabetes.
    Patients that have been diagnosed during the last 3 years may also attend providing they are not on Insulin or maximum orals.
    Referral is normally via the GP or Practice nurse. Download the Referral Form
