NHS Confederation and NHS Providers have just launched a new shared learning hub, the Community Network, collating examples of good practice from NHS Trusts and community interest companies providing NHS community health services.

The Community Network aims to facilitate shared learning across the country, and highlight the innovative work of the community sector. We are extremely proud to announce that SCFT services have already contributed five case studies to the hub:

Adapting Time to Talk

Moving from predominantly face-to-face services to remote appointments, leading to higher attendance rates for clinical appointments, as well as an overall increase in staff productivity.

Diabetes support

Optimising the Diabetes Care for You service to support patients with type 1 diabetes and ensure they have the information and resources they need.

Hospital at Home

Introducing remote health monitoring and the use of specialist devices to enable patients who need ongoing oxygen therapy to be cared for at home rather than in hospital.

Implementing a discharge hub

Working with local system partners to implement a multi-agency decision-making team and discharge hub to significantly reduce patient discharge delays.

Care home support team

Shifting from face-to-face to online training, providing bespoke virtual e-learning sessions via Zoom for care home staff on topics including catheter care, pressure areas and urinary tract infections.
