Supporting people approaching the final stages of their lives

Clinicians at Sussex Community NHS Foundation Trust (SCFT) have produced two guides to support patients, families and carers as people approach the final stages of life.

In the last year SCFT’s Clinical and support teams have cared for more than 1,500 people at the end of their lives.

We understand that the news that you or someone you care about is nearing the end of life can cause a range of emotions and concerns.

SCFT has produced a booklet to help patients, their families and carers to manage during a difficult time.

Lauren Smith, End of Life Care Lead at SCFT, said: “We wanted to make sure that we give people the information and support they need when they or a loved one is approaching the end of their life.

“Our booklets offer support, advice and signpost people to useful local and national services that might help them during a difficult time. Helping patients to access and understand information that may otherwise be hard to take in is really important.

“These resources also help our clinical teams communicate, to have better quality conversations with patients, their friends, families or carers.”

SCFT also runs a successful project in West Sussex that has joined up the services that support people at the end of their lives.

ECHO, the Coastal West Sussex End of Life Care Hub, improves the coordination and delivery of end of life care across Coastal West Sussex by linking key services via a 24/7 telephone coordination hub staffed by trained nurses.
