Who we are

Our Hospital Rapid Discharge Team (HRDT) supports and enhances the Accident and Emergency (A&E) experience for patients, and reduces admissions or readmissions to an acute hospital.

What we do

We provide input into the multi-disciplinary team at Accident and Emergency (A&E) to ensure that there is a rapid assessment for patients (including comprehensive geriatric assessment as appropriate), discussions for differential diagnosis, further investigations or interventions, and an appropriate discharge plan or follow up in the community.

How can I access this service?

We provide support to patients in an acute hospital setting only. We will see patients over 18 years old from any locality, who are medically ready for discharge from the acute hospital.
