What does this service provide?

We provide specialist palliative care support to patients and families affected by complex life limiting illness.

Specialist palliative care services are required for people living with more complex and/or long-term conditions which are life-limiting or life-threatening. The needs of this group cannot be met by their core team alone. Our team provides specialist advice and support to the wider care team providing direct palliative care to the patient.

Our Midhurst Macmillan service team includes:

  • service managers
  • doctors
  • clinical nurse specialists
  • clinical support team
  • counsellors
  • occupational therapists
  • physiotherapists
  • bereavement support team and volunteers

What we do

  • Provide patients with holistic care, addressing their physical, spiritual, social and psychological needs.
  • Support carers in their important role.
  • Provide patients with nursing care to support them to stay at home in comfort, with good symptom control.
  • Where possible, offer appropriate clinical interventions at home.
  • Provide access to consultant and doctor support for patients with complex needs.
  • Link in effectively with NHS, social care and other services involved in patient care and to complement and support these services in a patient centred approach.
  • Provide patients and their carers with strategies to manage their condition where possible.
  • Prevent unnecessary and inappropriate admissions, and expedite discharge from hospital.
  • Work as a multi-disciplinary team in partnership with service users, families and carers to promote and facilitate optimal health, independence and disease/condition management for individuals.

Appointment types

We offer a range of appointments, including face-to-face appointments, home visits and in person consultations at The Pearson Unit, Midhurst Community Hospital. We also provide telephone and video consultations.

Initially, you will be seen by a Clinical Nurse Specialist in Palliative Care.


Where can I access this service?

We accept patients 18 years and over who are registered with a GP within our catchment area. We accept patients with any diagnosis and a need for specialist palliative care.
