Who can contact OneCall?

Referrals can be made at any time by patients, relatives, carers, healthcare professionals, social services, ambulance services and acute trusts. Some referrals can only be accepted from Healthcare professionals. 

Referrals we receive may include support for patients to stay at home safely, support to aid a patient’s discharge or avoid hospital admission, and support with ongoing health conditions.  

All patients being referred must be over 18 years old and registered with a GP within West Sussex. For most of the community services that OneCall make referrals to, patients need to be housebound, although exceptions apply. 

We are not an emergency service, but we do aim to help every caller. If a referral is not appropriate for our service, we will always signpost if we can. 

Who you'll speak to 

A mixture of clinicians, nurses and care coordinators handle all our communications. When we receive a referral from you, you’ll be asked a series of questions regarding the reason for the call, and the referral will then be sent to the most appropriate team.
