Community Short Term Services (Brighton & Hove)

Sussex Community NHS health trust, Brighton and Hove Council, Brighton and
Hove Age UK, South East Health and Victoria nursing homes have been commissioned to work collaboratively for people in a crisis to provide care, where appropriate, to support patients at home rather than admission to a hospital setting.

Together the services can provide a rapid response from a multidisciplinary team for 72 hours from a range of professionals to prevent admission, access to a community bed for inpatient rehabilitation or an intensive period of rehabilitation in a patients own home setting.

Support can also be provided for night sitting and GP cover as and when needed. There are excellent links with Brighton and Sussex University for advice and support from care of the elderly consultants.

The services which make up the Community Short Term Service include:

  • Community Rapid Response (CRRS)
  • Roving GP service
  • Out of hours district nursing service
  • Age UK (Brighton and Hove) Crisis service
  • Bed and home based rehabilitation (formerly known as intermediate care)
  • Care of the elderly consultants

Since April these services have all been working together more closely and it is now only necessary for a patient to be assessed once and to make one referral even if multiple services are needed.

All referrals for Community Short Term Services should be made via the NHS 111
Professional Support Line (PSL) on 0300 130 3045 by a GP or health care professional

The PSL will take all the relevant information and forward it electronically to the Community Short Term Services Single Point of Contact. When received the referral will be signposted to the most appropriate service and responded to in a timely manner.

Community Short Term Services provide:

  • 2 hour response for urgent needs to prevent hospitalisation

  • GPs who are able to make urgent house calls

  • Multidisciplinary assessments to prevent hospital admission

  • Registered Nurses and nursing assistants who work in the evenings, and at night

  • Experienced teams of skilled nurses, occupational therapists, physiotherapists, rehabilitation assistants, care workers and social workers who support people at home with a programme of rehabilitation, or through a short crisis

  • Residential centres which provide rehabilitation and nursing support

  • Medical support from Care Of the Elderly Consultants and GPs

  • Access to Brighton and Hove Age UK to provide crisis support

The services are not new but are working more closely together to improve patients and referrers experiences.

Sussex Community NHS Trust provides the Community Rapid Response and Intermediate Care in the home services.

Access to the services for the public is through an assessment by a GP or health care professional and referral made via NHS 111 Professional Support Line.
