Shortlisted – KSS Service Improvement and Innovation Awards 2015
We’ve been shortlisted at the Kent, Surrey and Sussex (KSS) Service Improvement and Innovation Awards 2015, for the category Enhancing Innovation Award for our Coastal West Sussex DocoboWeb Pilot.
The pilot, organised by Sussex Community NHS Trust (SCT) and working together with doc@HOME and eight nursing/residential care homes within Coastal West Sussex, was run for eight months between November 2013 to June 2014 and achieved:
- 75% reduction in admissions to hospital with the same patients compared to the same period a year earlier.
- 49% of patients had no admissions throughout the pilot compared to the previous year.
- Improved knowledge, skills, competencies and confidence of staff within residential care homes, not just with patients on the pilot but for all residents in their care.
The project uses a lower intensity of Telehealth to assist in the early detection of signs/symptoms to prevent unnecessary admissions to hospital for people with a long-term condition including heart failure, diabetes, chronic obstructive pulmonary disease (COPD) and urinary tract infection.

We’ve learnt a lot from the SCT community matron about observations and how to recognise when someone is not well. This is so important to us given we are a dementia home and our residents respond better in their own environment”, New Grange Care Home.
