What does this service provide?

Who we are 

Our team is made up of nurses, community prevention officers and community support workers. Our teams include staff from West Sussex County Council, Sussex Community NHS Trust, and Guild Care or Age UK West Sussex. 

What we do 

We are here to help people in communities across West Sussex access the support they need to live independently at home and enjoy life as part of their community. We are also here to help prevent or delay the need for more intensive services.  

One of our team will:

  • advise you on how to keep healthy and manage health issues 
  • link you with agencies that can maximise your income and help you remain independent through practical tasks such as shopping, cleaning and gardening 
  • put you in touch with local social groups and activities 
  • help you keep your home safe, secure and warm 
  • let you know what transport services are available locally   

Our nurse advisors attend and set up community events offering blood pressure and pulse checks. We also provide NHS health checks in outreach settings by appointment.

Who you'll see at your appointment 

You will speak to, or be seen by, a nurse, a community prevention officer or a community support worker. 

What happens after my treatment or therapy plan ends? 

You will receive a telephone call from our team to review the support you've received.

We offer a variety of options, including telephone and home appointments. It's also possible to see our team at one our community outreach events.

How can I access this service?

There are several ways to access our services. We accept referrals from health and social care staff, well-being teams and any other statutory and voluntary sector partner, if they have been made with the patients consent.

We are open Monday to Friday, from 9:00am to 5:00pm. We are closed on weekends and bank holidays.
