The Trust Board has recently approved our 5 year transformation plan which describes how we convert our vision, strategic goals and service development plans into a deliverable programme of work.

Transformation is about reshaping our services to:

  • put patients at the centre of care,
  • proactively meet their health needs,
  • empower them to manage their own care,
  • deliver services in or as close to their home as possible,
  • and provide a sustainable business to secure the future of the Trust.

Transformation will build on the many good things about our services, but we cannot achieve our strategy and meet future challenges without change. Transformation will touch all services in the trust and many of our transformation programmes/projects involve working with partners such as GPs, Social Services, acute hospitals and other NHS trusts.

The plan will be delivered through a structured programme management process, and appropriate programme and clinical leads have been identified to lead the transformation process and ensure we deliver the benefits required.
